Job Title: CWHH-RN Home Health
Location: Lancaster, PA
Setting: Home Health
Pay Range: $36.99 – $38.84 per hour
Schedule: 40 Hours per week, Days, Variable shifts to meet operational needs
Contract Length: Discuss with Recruiter

Requirements:

• 2 years of experience as an RN
• Qualified applicants MUST have at least 2 years of experience in clinical care
• Qualified applicants MUST have current state RN license
• Must have Homecare Homebase Software experience – Required
• Must have OASIS experience – Required
• Must have current CPR certification – Required
• Must have reliable transportation and insurance – Required
• Proof of current CPR certification – Required
• Bachelors of Science in Nursing – Preferred
• CHPN certification – Preferred
• Be willing to obtain State licensure

Responsibilities: As a healthcare professional in a home health setting, you will…

• Assess home health patients to identify physical, psychosocial, and environmental needs.
• Develop, implement, and evaluate the Plan of Care (POC) to ensure quality and continuity of care.
• Educate patients and families on care, medications, and safety.
• Supervise and provide clinical directions to home health aides and LVNs.
• Monitor and reassess pain, utilizing appropriate pain management techniques.
• Arrange referrals to appropriate agencies when needed.
• Maintain accurate, up-to-date patient records reflecting all visits and care plans.
• Perform on-call responsibilities and provide support to patients and families after hours.
• Communicate effectively with patients, families, interdisciplinary team members, and healthcare professionals.
• Attend team meetings to coordinate care and improve patient outcomes.
