Job Title: PT Home Health Therapy/Rehabilitation
Location: Dover, DE
Setting: Home Health
Pay Range: Discuss with Recruiter
Schedule: 40 Hours per Week (Day, 08:00 – 17:00)
Contract Length: 13 weeks
Candidate Type: Local candidates only – travel stipend not available

Requirements:

• Minimum 1 year of recent Adult Home Health experience
• Qualified applicants MUST have current CPR and BLS certifications
• Valid medical license in Delaware (must have in hand unless in process)
• Willing to travel up to 75 miles from the Dover clinic; mileage reimbursed at IRS standard rate
• Weekend work varies per location and profession; discuss during interview
• On-call required? No
• Certs required: CPR, BLS

Responsibilities:
As a healthcare professional in a home health setting, you will:

• Provide skilled physical therapy services (PT) to adult patients in their homes, enhancing mobility and independence.
• Document patient progress and treatment plans efficiently using HCHB EMR system.
• Coordinate care with family members and healthcare teams to ensure continuity of services.
