Job Title: Physical Therapist: Home Health

Location: Fernandina, FL (office located in Jacksonville, FL)
Setting: Home Health
Pay Range: $2200-2400/weekly (or hourly)
Schedule: 8-Hour Days
Contract Length: 13 Weeks
Candidate Type: Local and traveler friendly

Requirements:

• 1 year of experience as a PT
• Qualified applicants MUST have at least 6 months of experience in Home Health
• Valid Physical Therapist license
• Be willing to obtain FLorida licensure

Responsibilities

• Deliver one-on-one, high-touch care to patients in their homes
• Conduct skilled nursing visits and perform assessments
• Create and manage individualized care plans
• Collaborate with families and interdisciplinary providers to improve patient outcomes
