Job Title: CWHH-RN Home Health
Location: Duluth, GA
Setting: Homecare
Pay Range: Discuss with Recruiter
Schedule: 40 Hours per week, Day shift (Note: Rotating, D/N, or variable shifts to meet operational needs)
Contract Length: 13 Weeks
Candidate Type: Traveler friendly - travel reimbursement available

Requirements:

• At least 2 years of experience as a RN
• Qualified applicants MUST have a current Georgia RN license
• Must have home health experience of 2+ years
• Knowledge of medications, venipuncture, IV care, wound care, inhalation therapy, and phlebotomy
• Recent BLS/CPR certification (Proof required)
• Current driver's license and reliable transportation
• Must have homecare software experience (Homecare Homebase) and OASIS experience
• Must be willing to obtain and maintain CHPN certification (preferred)
• Ability to respond to patient needs effectively in varied settings
• Effective communication skills and ability to work independently and as part of a team

Responsibilities:
As a healthcare professional in a homecare setting...

• Administer skilled nursing care and develop, implement, and evaluate the Plan of Care (POC).
• Educate patients and families on care, safety, medications, and self-care practices.
• Provide clinical supervision and direction to Home Health Aides and LVNs.
• Assess and manage pain and monitor patient progress.
• Arrange referrals to appropriate services when needed.
• Maintain accurate documentation of visits, interventions, and patient progress.
• Perform on-call duties and respond promptly to patient needs.
• Communicate effectively with the interdisciplinary team, patients, and families.
• Attend team meetings and perform other related duties as assigned.
